Catholic Charities is seeking an energetic, professional, and supportive Care Management Coordinator to join our team. The Care Management Coordinator will oversee quality assurance measures and client care coordination in our residential program in the Madison area serving youth and adults with developmental disabilities and/or severe emotional disturbance.
Our ideal candidate will have at least 5 years’ experience in residential programing including supervision and managing services for youth and/or adults in community settings. Our Care Management Coordinator will ensure Catholic Charities policies and procedures, state, and Family Care regulations are being met and will offer guidance and support to program managers around client services. Other responsibilities of the position include: evaluate each program participant’s health/safety and risk level and provide recommendations to teams, assess new referrals and complete the placement process, and monitor fiscal budgets. This position entails working 37.5 hours a week Monday through Friday and occasional additional hours may be required.
